Pre-appointment details

At Pulse Physical Therapy & Performance, we strive to provide the best care for our clients. Before your appointment, we kindly request that you arrive 15 minutes early if you have not filled out the forms provided above. This will ensure that we have enough time to properly assess and address your needs.

Cancellation policies

We understand that unexpected circumstances may arise, and you may need to cancel your appointment. In order to better serve all of our clients, we kindly ask for at least 24 hours notice for cancellations. This allows us to offer the appointment slot to another client in need. If cancelations occur within 24 hours you will be subject to a $50 cancelation fee. Thank you for your understanding and cooperation.

Payment policies

At Pulse Physical Therapy & Performance, we accept various forms of payment for our services. Whether you prefer cash, check, or credit card, HSA, or FSA we strive to make the payment process as convenient as possible for you.
